La Dermatite Seborroica
La dermatite seborroica è uno stato infiammatorio della cute, coinvolge quasi sempre il volto, il cuoio capelluto, lo sterno, il dorso e il condotto uditivo, un disturbo comune soprattutto nei soggetti di sesso maschile di età compresa tra i 30 – 40 anni. La dermatite seborroica ha un andamento cronico-recidivante, le cause sono molteplici: la poca esposizione al sole o il freddo, oppure stress emotivo, deficit immunitario, farmaci.
Nella zona del cuoio capelluto potrebbe esserci una modesta desquamazione mentre sul viso si presenta a vaste zone cutanee, con infiammazione, prurito e desquamazione.
Questa patologia è legata alla ghiandola sebacea, un’alterazione della regolazione sebacea da cui prende il nome “seborroica”.
Che funzione ha il sebo?
Il sebo è prodotto quindi dalle ghiandole sebacee e negli uomini serve per svolgere una funzione protettiva della cute all’ambiente esterno, il sebo è una sottile pellicola che protegge la sottostante epidermide dal freddo, dalle radiazioni UV, dalla perdita di acqua transepidermica, dall’ingresso di patogeni esternicopre.
Le aree a maggior densità ghiandolare come:
- La fronte;
- Ali del naso;
- Padiglione auricolare.
Le infiammazioni non si manifestano nel periodo della pubertà perchè la secrezione sebacea è regolata dagli androgeni circolanti.
Si può notare nei neonati perché la crosta lattea ha origini dagli androgeni materni e scompare dopo le prime settimane.
Nei mammiferi ha una funzione di tipo sessuale (feromoni).
Da cosa dipende?
Ci sono molti fattori che provocano la comparsa della dermatite seborroica, fattori che spesso vengono messi in discussione. Da recenti studi è emerso che la dermatite seborroica può dipendere da vari fattori:
- La qualità del sebo;
- Da un fungo del genere Malassezia;
- Dalla suscettibilità individuale;
- Scarsa igiene;
- Variazioni ormonali.

Le Terapia
Esistono terapie etimologiche e sintomatiche.
Le terapie sintomatiche prevedono farmaci per la riduzione della flogosi (corticosteroidi topici), per la riduzione della carica fungina (antimicotici) e per la rimozione del sebo e della forfora (saponi, detergenti, etc), nessuna terapia è in grado di prevenire le recidive o allungare i tempi di ricomparsa delle recidive.
La dermatite seborroiica va trattata con sostanze nella fase acuta e terapie di mantenimento per prevenire le recidive. Di solito le terapie efficaci sono quelle di tipo sistemico perché forniscono una protezione antiossidante che intervengono a livello del metabolismo lipidico e della sintesi degli acidi grassi.
La biotina ha le caratteristiche ideale per il trattamento della dermatite seborroica perché riesce a :
1) modificare il profilo lipidico (qualità del sebo);
2) ridurre lo stress ossidativo a livello cutaneo (attivazione immunitaria);
3) inibire il rilascio di citochine infiammatorie (flogosi cutanea).
Da recenti studi è stato evidenziato che la biotina costituisce l’unico farmaco a migliorare l’infiammazione nel trattamento acuto,
somministrando dose di 10 mg al giorno per almeno due mesi e può aiutare a ridurre la frequenza delle recidive ed allungare i tempi di ricomparsa delle stesse, se somministrata con una specifica schedula di trattamento ciclico annuale neo mesi di:
